The diet of many Americans remains unhealthy, contributing<br />
to high rates of childhood and adult obesity that are associated<br />
with health outcomes such as heart disease and stroke. To<br />
promote wellness, the healthcare industry must go beyond<br />
treating individuals with chronic conditions to also address<br />
the risks of di erent population segments before they reach<br />
advanced stages of illness.<br />
Population Health Management (PHM) is helping to shi the focus<br />
towards wellness using a broader perspective that promotes<br />
improved health outcomes for entire groups or populations.<br />
