MANAGEMENT (PHM) DEFINED<br />
PHM is a broad effort whereby individual-, organizational-<br />
and cultural-level interventions are planned and<br />
implemented to improve the disease burden of a<br />
particular geographical area. Further, PHM strategies<br />
are used to decrease overall health care usage or to<br />
encourage primary prevention efforts to prevent future<br />
overuse of the health care system by keeping people well<br />
at the onset. PHM is a systematic effort at stratifying<br />
populations across health-risk profiles and applying<br />
